This specimen document is a copy of the insurance policy available for clients who have booked their travel arrangement at Bath Travel.
Cover will only be provided for each passenger who is shown as having paid the insurance premiums and whose name is shown on a receipt issued by Bath Travel.
Declaration Form
It is your responsibility to provide complete and accurate information to insurers when you take out the insurance policy and to notify any changes in your health or medication between booking and travel.
All clients purchasing insurance from Bath Travel must read the Declaration Form and sign it to confirm acceptance.
